Any student/family wishing to withdraw from CSArts-SGV mid year, should fill out the appropriate form below. If you plan to finish the year and do not plan to return to CSArts-SGV in the fall, we also request that you fill out the appropriate form below as that alerts us to important enrollment information. This can be done prior to the last day of attendance.
